I've noticed that I wasn't always getting a Time Halo equipped from maximize adv, despite having no other +adv items for hands or that accessory slot.
Time Halo is unusual in that it's a +adv accessory only if your hands are empty.
I tried testing this a little, and found something a little strange - it *sometimes* works.
Starting from naked, it works:
But running another maximize statement before it (I just did this to get a full set of gear, what it is doesn't matter) breaks it:
After messing around a bit more I think the conditions are that it will use the Halo properly in two situations:
1) Halo is already equipped, in which case it will empty the hands so it works.
2) Hands are already empty, in which case it will equip the halo.
But if the hands are full and the halo is not equipped, maximizer can't get to it.
