Azazel quest gone?

One of my scripts (ascend) started behaving oddly, and I found the problem: my main, dj_d, no longer has the "azazel, ma belle" entry in his questlog. He's level 42 and still has a liver of steel (he got it in 2004 ), and my multies still have their entry, but for some reason it's missing on him. I believe this happened recently, but can't be sure.

Any ideas what might have happened?

That quest is "Angry <playername>, this is Azazel in Hell."in the completed quest log.

Right. In everyone's quest log except my main's. (I realize I confusingly referenced the quest by it's other name in the first post, but either way - it's not there).
