When you drop HC, mafia thinks that you are free of restrictions, even if you are still in Ronin. I think this would be fixed when the charpane next loads (not sure), but people seem to be dropping HC (to wrap up a failed HC run and try again I guess) and then immediately using Unlucky Entryway. This results in mafia using all pulls on the first item wanted for the tower.
Assuming I understand what the code is doing, the attached patch should make mafia immediately aware of your ronin state when you drop HC. It also updates your ronin state when you drop softcore.
